If you are returning to continue a partially completed form, check box at left for instructions.

If you accessed this screen using the custom link (including the phrase "resumecode") that was displayed and emailed to you after logging out, simply reenter the password originally issued to you by your health practitioner above and click the 'Proceed' button.  The system should remember you and return you to the last page you were on.

If you do not have the custom "resumecode" link are attempting to continue a form using the link originally issued by your health practitioner, reenter the original password above and click 'Proceed.'  After agreeing to the Terms of Use on next page, reenter your name and email address on page 1 of the form exactly as you did on the previous occasion.

The system should then recognize you and display a message asking whether you would like to continue the form where you left off.  Choose the option "Resume Previous Form" to be returned to the page you were last on.

Note: You cannot return to and modify a form you have already successfully completed.
