Types of Forms

Depending on your requirements, you can choose from 3 forms:

Extended — Approximately 100 questions, 15-30 minutes to complete
Standard — Approximately 75 questions, 10-25 minutes to complete
Short — Approximately 50 questions, 5-15 minutes to complete
Custom Forms — You can choose any questions from our extended form, or create your own questions , in order create a form perfect for you.

All forms are the same price.

To see all the questions for each form, click here and enter the password “demo.“ You will be able to answer questions to see how the form adjusts itself, such as changing questions to match the client’s gender and age, or to see how a question asks for more details when a significant issue is identified.

Standard Form

Average 10-25 minutes to complete.

Approximately 75 questions about the following topics:

• Client’s living situation (15 questions)
• Birth (9 questions)
• Medical history (20 questions)
• Developmental milestones (8 questions)
• Sensory Issues (8 questions)
• Educational history (9 questions)
• Behavioral/psychological issues (6 questions)
• Prior diagnosis (2 questions)

Custom Form

You can create your own form by:

• Choosing any of the questions from the extended form by going to this page — delivery within two weeks
• Creating your own questions by going to this page a?? we will assist you to design questions and format their report responses

The cost to do this will depend on the number and nature of the question(s), the amount of programming required, and amount of credits you purchase to use with the new form.
